What is meant by Proof of Claim?
An individual with a full medical diagnosis for a silica-related disease dated prior to February 12, 2002, or a chest x-ray or
CT scan that was read or interpreted prior to February 12, 2002 and found to be positive for a silica-related disease compensable
by the Kaiser Silica PI Trust is barred from allowance and payment of his/her claim by the Trust unless a Form B10, also known as
a Proof of Claim, was filed with the Claims Agent, Logan & Company, Inc., on or before the Silica Bar Date of 1/31/2003.

How much time do I have to file a claim after my diagnosis?
In accordance with the Silica Distribution Procedures, all Silica Personal Injury Claims which were first diagnosed on or after
February 12, 2002 must be filed with the Trust by December 31, 2009, or three (3) years after the date of diagnosis, whichever
is later, to be considered for allowance and payment.
